We have successfully received your application and have sent a Prosthetist Verification Form to the prosthetist you listed in your application.

What Happens Next?

1. Your prosthetist will complete and submit the verification form directly to Walk-Run-Play.
2. Once we receive the completed verification form, our team will review your application and supporting documentation.
3. If any additional information is needed, we will contact you using the email address or phone number provided in your application.
4. After the review process is complete, you will be notified regarding the status of your funding request.

Please note that application review cannot begin until the Prosthetist Verification Form has been received.
